Following your breakfast event send* in your sales receipt, UPC code cut out of each bag of pancake mix used and the following contact information (or download and print out form below)

• Date of Pancake Breakfast • Mailing Address • Number of People Served
Continental Mills will reimburse your organization 50% of the purchase price of Krusteaz® Pancake Mix — up to $100.00 (Any sales tax must be paid by the organization.)
